SUMMARY:
Primary responsibility of the Family Care Coordinator (FCC) is to coordinate, conduct and manage services for individuals/families approved for Autism Respite Care though the Department of Human Services – Division of Family and Children Services by coordinating temporary Autism Respite Care Services. The coordinator is responsible for case management and supervision of Autism Care Respite Direct Support Professionals and Autism Care Individuals/Families. The FCC is also responsible for coordinating and managing services for Family Support Service
Essential Duties and Responsibilities – Includes the following:
Provide direct placement services and supervision following placements, provides case management and monitoring ARC placements.
Creates and supervises ARC-DSP schedules.
Provides first point of contact from referral sources for intake.
Coordinates intake and discharge of individuals in ARC or Family Support Respite.
Creates and implement care plans and discharges.
Tracks, maintains, and meets annual goals.
Regular 1:1’s with defined frequency from Family Care Manager
Provides on call support or Provides Autism Respite Care when necessary (staff call out, unable to find staff)
Develops programmatic systems for maximum efficiency.
Communicates effectively with the other departments as well as DHS - DFCS
Implements Quality Assurance tools.
Provides ongoing recruitment of ARC DSPs, and Family Support Service Providers
Assists in the preparation of programmatic reports.
Coordinates and issues voucher request for respite
Educates, provides customer support, and empowers families about service options available.
Operates within a team approach.
Adheres to InCommunity’s policies concerning confidentiality of individual’s information.
Adheres to InCommunity’s documentation standards for participant and provider records.
Other duties as assigned.

QUALIFICATIONS:

EDUCATION:
Preferred B.A./B.S. Degree or equivalent. One year of coordination experience in development disabilities. Experience in effectively implementing behavior or similar programs.

To perform this job, an individual should have knowledge of Spreadsheet software and Word Processing software, database management and administration, general knowledge of report writers, general knowledge of network administration.
Reasoning Ability
Ability to apply common sense understanding to conduct instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Physical Demands
While performing the duties of this job the employee is frequently required to sit. The employee is occasionally required to stand walk; use hands to finger, handle or fell reach with hands and arms; climb or balance, stoop; kneel crouch, crawl, talk or hear; and taste or smell. The employees must occasionally lift and/or move/ up to 25 pounds or more as required. Physically able to bend/ stop. Squat, push/pull, reach above and below shoulder level frequently. Physically able to occasionally kneel, walk on uneven ground and crawl. Specific Vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Qualifications
Salary : $44,000
