The RN Care Coordinator will ensure the delivery of gender-affirming care, assist in managing chronic conditions, and advocate for overall wellness.
Key Responsibilities
- Care Coordination & Support: Deliver comprehensive management for individuals living with HIV/AIDS, applying evidence-based practices aligned with the Patient-Centered Medical Home (PCMH) model.
- Collaborative Care: Engage with clinical teams to identify patients requiring care coordination, particularly those receiving HIV and gender-affirming care, while also facilitating referrals and medication education.
- Patient Engagement & Education: Organize educational initiatives for patients, including quarterly Client Advisory Board (CAB) meetings, and provide continuous HIV-related education.
- Resource Referral: Connect patients with behavioral health services, substance use treatment, specialty care, and community resources that address SDOH.
- Medication & Health Management: Aid patients in comprehending their medication regimens and health management strategies to support viral suppression and enhance overall well-being.
- Collaboration with Care Teams: Work alongside medical case managers and interdisciplinary team members to ensure efficient and comprehensive care for HIV-positive patients throughout the care continuum, including STI testing, PrEP, and PEP services.
- Compliance & Safety: Maintain adherence to HIPAA, corporate compliance, and all pertinent local, state, and federal regulations to uphold high standards of patient care and organizational integrity.
- An Associate’s degree in Nursing from an accredited institution, in addition to a valid Registered Nurse (RN) license in New York State.
- A minimum of two years’ experience in a healthcare setting.
- Strong communication and interpersonal skills, demonstrating an ability to provide exceptional customer service.
- Proficiency in utilizing computers and healthcare software systems.
- Bachelor’s degree in Nursing and/or two years of experience working with patients suffering from chronic conditions.
- Experience in care coordination and familiarity with the HIV demographic is highly preferred.
- Bilingual capabilities and training in laboratory/phlebotomy techniques are advantageous.
