Key Responsibilities
- Deliver patient and caregiver education on diagnoses, medications, infection control, and safety protocols as specified in the care plan.
- Manage administrative on-call duties during after-hours, weekends, and holiday rotations.
- Supervise weekend schedules from 8 AM to 5 PM, ensuring productivity and managing admissions efficiently.
- Facilitate weekend workflow and assist in coordinating between clinical teams to improve patient outcomes.
- Ensure adherence to state, local, and payer requirements, while verifying that documentation meets all regulatory standards.
- Oversee the procurement and management of equipment and supplies to enhance client independence.
- Exhibit competency in therapeutic interventions and educational techniques across varying levels of patient care complexity.
- Provide leadership and direction to weekend clinical staff.
- Educate patients and caregivers on safety measures and updates regarding patient status.
- Maintain precise, thorough, and comprehensive reporting in alignment with agency and regulatory standards.
- Review and uphold accurate OASIS documentation within designated timeframes.
- Report incidents in accordance with agency and regulatory protocols.
- Comply with policies and procedures as per state regulations and nursing standards.
- Engage in staff meetings, patient care conferences, in-service training, and other designated meetings.
- Ensure confidentiality of patient information and uphold patient rights.
- Execute additional responsibilities as assigned within the scope of practice.
- Nursing degree from an accredited institution.
- Valid RN license in the state of practice.
- Current CPR certification.
- Previous experience in home care preferred.
- Strong clinical competencies.
