Key Responsibilities
- Act as a patient advocate, assisting individuals in navigating the complexities of the healthcare system.
- Engage in quality improvement initiatives aimed at enhancing patient outcomes.
- Identify and address gaps in care by coordinating services and ensuring appropriate follow-ups.
- Collaborate with external healthcare facilities to ensure continuity of care during transitions, including admissions, discharges, and specialty referrals.
- Support patients and families during transitions of care, particularly following hospital visits and referrals.
- Co-manage care for high-risk patients, especially those with multiple chronic conditions, in collaboration with the Clinical Care Management team.
- Tackle Social Determinants of Health (SDOH) by evaluating screenings and providing community-based resources to those in need.
- Maintain effective communication with the healthcare team through thorough chart documentation and team discussions.
- Contribute to the collection of data and health outcomes reporting related to Patient-Centered Medical Home initiatives.
- Aid in the formulation of clinical tools and procedures to boost patient care efficiency.
- Active LPN certification (CMA experience is an advantage).
- Strong communication skills essential for educating patients and collaborating with their families.
- Proficiency in Microsoft Office (Excel, Word) and intermediate computer skills.
- Excellent interpersonal abilities and customer service skills, fostering a positive team environment.
- Strong problem-solving capabilities, with the ability to effectively escalate issues to providers when required.
- Self-motivated, detail-oriented, and reliable.
- Capacity to manage a caseload of approximately 300 patients monthly.
- Ability to thrive in a fast-paced environment and adeptly handle stress and conflict.
- Commitment to maintaining confidentiality and adhering to all relevant policies and procedures.
- A minimum of 1-2 years of relevant experience in case management or a comparable healthcare role.
- Familiarity with electronic medical records (EMRs); experience with Athena is a plus.
- Proven success in collaborating with medical providers to coordinate patient care plans.
- Experience in engaging with insurance companies to obtain patient data and resources.
