- Conducting thorough assessments to identify the unique needs, preferences, and goals of each individual.
- Developing and maintaining tailored Life Plans that reflect both clinical and non-clinical healthcare needs.
- Promoting health and wellness by ensuring access to resources for chronic disease management and long-term care.
- Collaborating with a multidisciplinary team to deliver comprehensive, person-centered support.
- Facilitating referrals to community resources and transitional care services.
- Advocating for individuals while fostering self-advocacy and independence.
- Utilizing technology to effectively document and communicate with the care coordination team.

To excel in this role, you will need:
- Strong time management and organizational skills.

- A bachelor’s degree accompanied by two years of relevant experience, or a master’s degree with one year of pertinent experience, or a valid RN license with two years of relevant experience.
