Compensation And Benefits
- Competitive Pay: \(22.50 - \)24.50 per hour
- Consistent Schedule: Monday to Friday, 8-hour shifts, with no weekends
- Generous Paid Time Off: 25 days annually

As a Care Manager, you will be instrumental in ensuring coordinated care and services for individuals with I/DD, enabling them to achieve their health, wellness, and life aspirations. Your responsibilities will include:
- Conducting thorough assessments to determine individual needs and preferences.
- Addressing health and safety issues, including social determinants of health.
- Collaborating with interdisciplinary teams to formulate personalized Life Plans.
- Monitoring and revising Life Plans to guarantee effective service delivery.
- Facilitating access to chronic disease management and long-term care resources.
- Involving families and natural supports in the care coordination process.
- Advocating for individuals and fostering self-advocacy.
- Maintaining precise documentation utilizing health information technology.
- Ensuring adherence to all relevant laws and regulations.
- Participating in continuous training and professional development.
To excel in this role, candidates must demonstrate:
- The ability to respond efficiently in crisis situations.
- Proficiency in utilizing basic technology and health records.
- A robust understanding of ethical and professional boundaries.
- Excellent time management skills, independence, and responsibility.
- Bachelor's degree coupled with two years of relevant experience, OR
- A valid license as a Registered Nurse with a minimum of two years of relevant experience, OR
- A Master's degree with at least one year of relevant experience.
- Previous experience working with individuals with Intellectual/Developmental Disabilities is essential.
