Key Responsibilities
- Conduct thorough assessments to evaluate clinical, psychosocial, and service needs.
- Identify health and safety concerns while addressing barriers to care, including social determinants of health.
- Collaborate with an interdisciplinary team to design and sustain personalized Life Plans.
- Coordinate and facilitate care services, including chronic disease management and community resource referrals.
- Provide guidance for transitional and long-term care services.
- Involve families and natural supports in the care coordination process.
- Advocate for individuals while promoting self-advocacy and self-direction.
- Utilize Health Information Technology for service documentation and communication with the care team.
- Ensure adherence to confidentiality and compliance with HIPAA and relevant regulations.
- Engage in continuous training and professional development.
- Proficiency in both English and Spanish.
- Basic technological competency, including familiarity with health records.
- Excellent organizational and time management abilities, with a capacity to work independently.
- A Bachelor's degree and a minimum of two years of relevant experience, OR
- A Registered Nurse (RN) license with two years of relevant experience, OR
- A Master's degree with at least one year of relevant experience.
- Experience in direct care, case management, or advocacy is highly desirable.
