Key Responsibilities
- Conduct monthly care coordination calls with patients, healthcare providers, pharmacies, and other relevant stakeholders to guarantee necessary support for patients.
- Collaborate with healthcare providers and patients to formulate and update personalized care plans.
- Identify and address social determinants of health and care gaps, referring patients to the appropriate resources and services.
- Encourage patients to actively engage in their own care and wellness objectives.
- Make approximately 30-50 calls daily while ensuring productivity goals are met and providing outstanding patient service.
- Uphold patient confidentiality while complying with legal and ethical standards.
- A minimum of one year of experience as a Certified/Registered Medical Assistant.
- Bilingual proficiency in Spanish and English with excellent verbal and written communication abilities.
- A solid technological proficiency, ideally with experience in multiple EMR/EHR systems along with familiarity in Microsoft Office and Google Suite.
- A professional and empathetic demeanor, capable of effectively connecting with patients and providers through phone and written communication.
- Strong organizational skills, self-motivation, and the ability to work autonomously.
- Previous experience in call center environments, triage, case management, or care coordination is advantageous.
- Certification as a Medical Assistant is required.
- Fluency in both Spanish and English is essential.
- Strong written and verbal communication skills are crucial.
