Key Responsibilities
- Perform comprehensive assessments to evaluate an individual’s medical, social, and emotional requirements.
- Create and sustain person-centered Life Plans that encompass both clinical and non-clinical services.
- Coordinate care services that include medical, behavioral health, and community resources.
- Advocate for individuals, fostering self-advocacy and independence.
- Connect individuals with community and healthcare resources through referrals.
- Promote health and wellness initiatives.
- Ensure adherence to HIPAA, Medicaid, and state regulations.
- Document and track provided services using Health Information Technology.
- Collaborate effectively with families and engage natural support networks.
- Strong organizational and time management abilities.
- Experience working with individuals with intellectual or developmental disabilities.
- Familiarity with healthcare systems, case management, or social work practices.
- Capacity to work both independently and collaboratively as part of a team.
- Basic proficiency in technology, including electronic health records and documentation tools.
- Bachelor's degree with two years of relevant experience; or a Registered Nurse (RN) license with two years of experience in any field; or a Master's degree with one year of relevant experience.
- Valid driver’s license is required.
- Willingness to commute or relocate to Binghamton, NY prior to commencing employment.
