In this fulfilling role, you will collaborate with individuals and their families to coordinate essential services, advocate for their needs, and assist them in achieving their health and personal goals. Key Responsibilities
- Conduct thorough assessments to identify each individual’s clinical and psychosocial requirements.
- Develop and sustain personalized Life Plans that reflect individual goals, health needs, and available community resources.
- Monitor ongoing progress and provide continuous support to ensure the effective execution of Life Plans.
- Advocate for individuals by facilitating their access to healthcare, community services, and long-term support systems.
- Address social determinants of health and eliminate barriers to care.
- Collaborate with interdisciplinary teams, families, and natural supports to enhance the overall well-being of individuals.
- Utilize Health Information Technology for documenting services, tracking progress, and enhancing communication among care teams.
- Ensure all provided services are culturally and linguistically relevant.
- Maintain compliance with state and federal regulations, HIPAA standards, and agency protocols.
- Engage in professional development and training opportunities.
- Strong organizational and time management abilities.
- Proven capacity to work independently and collaboratively within a team.
- Compassionate and dedicated approach to service coordination and advocacy.
- Basic proficiency in technology for managing electronic health records.
- A Bachelor's degree accompanied by at least two years of relevant experience, or
- A Registered Nurse (RN) license with two years of relevant experience (this may include various employment experiences, not limited to case management), or
- A Master's degree with a minimum of one year of relevant experience.
- Prior experience working with individuals with intellectual and developmental disabilities is a prerequisite.
