Key Responsibilities
- Manage a caseload of post-release services, ensuring adherence to program policies and timelines.
- Collaborate with care providers and other stakeholders to evaluate the appropriateness of sponsors and living conditions for children.
- Conduct virtual and in-person home visits, employing trauma-informed practices and brief therapeutic techniques to engage with children and their sponsors.
- Deliver ongoing case management, which includes assessments, planning, and psychoeducation to foster a supportive environment for children.
- Maintain comprehensive and precise documentation, ensuring the confidentiality of sensitive information.
- Raise any concerns regarding the safety and well-being of children and sponsors to the appropriate authorities.
- Participate in training sessions, meetings, and quality improvement initiatives to remain aligned with program objectives.
- Travel as required to perform home visits across various regions in the U.S., including occasional evenings and weekends.
- Strong verbal and written communication abilities, fluency in both English and Spanish preferred.
- Proficiency in composing detailed and professional assessment reports.
- In-depth knowledge of child welfare policies, family preservation, kinship care, and trauma-informed care practices.
- Excellent computer skills and experience with Microsoft Office and data entry systems.
- Proven ability to navigate complex situations with empathy and effective problem-solving skills.
- Bachelor's degree in Social Work (BSW) or a related field such as psychology, sociology, or social services.
- Minimum of 1 year of experience in case management, ideally with immigrant populations.
- Demonstrated expertise in trauma-informed care and child welfare practices.
- A valid driver's license and willingness to travel for home visits.
