Join our dedicated team as a Case Manager Level II, where you will play a pivotal role in our Post Release Services program. This remote opportunity empowers you to make a significant difference in the lives of children and their families by ensuring safe and supportive placements.

Key Responsibilities

  • Manage a diverse caseload of post-release services cases, emphasizing family preservation and child welfare.
  • Conduct detailed home studies to evaluate safety, risk, and placement appropriateness.
  • Provide continuous case management, including assessments and psychoeducation for children and their sponsors.
  • Collaborate with local resource providers to gather essential information for thorough evaluations.
  • Utilize trauma-informed care and brief therapeutic techniques to cater to the needs of children and their families.
  • Maintain consistent communication through virtual and in-person home visits.
  • Ensure meticulous and secure documentation in accordance with program regulations.
  • Act as a mandated reporter, addressing any signs of abuse or neglect.
  • Engage in training sessions and quality improvement initiatives.
  • Travel across the country for in-person visits, which may include evening and weekend commitments.

Qualifications

  • A Bachelor's degree in Social Work, Education, Psychology, Sociology, or a related field.
  • Minimum of 1 year of experience in case management.
  • Competence in crafting detailed, professional assessment reports.
  • Experience with immigrant populations and knowledge of trauma-informed care practices.
  • Understanding of family preservation, kinship care, and child welfare policies.
  • Bilingual proficiency in English and Spanish (both verbal and written).
  • Adeptness in Microsoft Office applications and data entry systems.
  • Valid driver’s license and willingness to travel nationwide.
