- Perform telephonic triage and assessments, delivering prompt support across various topics, including behavioral health, substance abuse, relationship issues, and work-life balance challenges.
- Manage and coordinate case intakes, ensuring each member receives personalized and impactful support.
- Execute clinical decisions in accordance with risk management policies, prioritizing both member welfare and organizational standards.
- Refer members to a broad network of service providers for additional assistance, encompassing financial counseling, legal support, and more.
- Offer coaching, brief solution-focused counseling, and necessary follow-up care via phone or telehealth platforms.
- Collaborate with service providers and engage with supervisors to ensure seamless integration of care and support services.
- Maintain professional and accurate documentation using a computer-based case management system.
- Provide consultative support to Human Resources and supervisory personnel concerning employee-related matters.
- Strong competencies in counseling and assessment methodologies.
