Key Responsibilities
- Conduct thorough assessments to identify individual needs, preferences, and goals.
- Address health and safety issues while recognizing barriers to accessing care and services.
- Develop and uphold a personalized Life Plan that meets both clinical and non-clinical requirements.
- Monitor individual progress and adjust plans as necessary to promote optimal health and well-being.
- Facilitate access to healthcare, chronic disease management, and long-term care services.
- Ensure effective transitions of care through appropriate referrals and follow-up actions.
- Promote health education and encourage individuals to pursue their wellness objectives.
- Advocate for individuals, enabling them to self-advocate and make informed choices.
- Maintain precise documentation and adhere to HIPAA, Medicaid, and relevant regulatory standards.
- Experience working with individuals with intellectual or developmental disabilities is essential.
- Strong organizational and time management abilities.
- Aptitude for quick assessment and decisive action in crisis situations.
- Familiarity with technology and health records systems.
- Bachelor's degree with two years of relevant experience, or a Master's degree with one year of relevant experience, or an RN License with two years of relevant experience.
- Valid RN License (if applicable).
- Possession of a valid driver's license and reliable transportation.
