Job Overview

Key Responsibilities

• Conduct thorough assessments to identify individual needs, preferences, and goals.
• Address health and safety issues while recognizing barriers to accessing care and services.
• Develop and uphold a personalized Life Plan that meets both clinical and non-clinical requirements.
• Monitor individual progress and adjust plans as necessary to promote optimal health and well-being.
• Facilitate access to healthcare, chronic disease management, and long-term care services.
• Ensure effective transitions of care through appropriate referrals and follow-up actions.
• Promote health education and encourage individuals to pursue their wellness objectives.
• Advocate for individuals, enabling them to self-advocate and make informed choices.
• Maintain precise documentation and adhere to HIPAA, Medicaid, and relevant regulatory standards.
  
  

Required Skills

• Experience working with individuals with intellectual or developmental disabilities is essential.
• Strong organizational and time management abilities.

Qualifications

• Bachelor's degree with two years of relevant experience, or a Master's degree with one year of relevant experience, or an RN License with two years of relevant experience.
• Valid RN License (if applicable).
• Possession of a valid driver's license and reliable transportation.
