Key Responsibilities

In This Position, You Will

• Respond to inquiries and perform comprehensive in-person nursing assessments for new clients.
• Develop tailored care plans and oversee the effective onboarding process for clients.
• Collaborate with caregivers to ensure they are knowledgeable and confident in executing care plans.
• Maintain consistent communication with clients, families, and caregivers to monitor progress and make necessary adjustments to care plans.
• Cultivate trusting relationships with clients and families while promoting service expansion within the local community.

Qualifications

Candidates must have:

• A Bachelor's or Associate's Degree in Nursing from an accredited institution.
• A valid Registered Nurse license in Maryland.
• A minimum of two years of nursing experience, with prior case management experience considered advantageous.
