Key Responsibilities
- Deliver a welcoming introduction to our services by promptly addressing new inquiries and conducting in-person nursing assessments.
- Lead the coordination of care by developing robust care plans, matching caregivers to clients, and guiding caregivers in understanding client needs.
- Maintain proactive and regular communication with all stakeholders involved in the care process to ensure alignment and satisfaction.
- Adapt care plans as necessary to reflect the evolving needs of clients and their families.
- Cultivate and nurture trusting relationships with clients, their relatives, and community referral sources, ensuring a highly positive experience.
- Bachelor’s or Associate’s Degree in Nursing from an accredited institution.
- Valid Registered Nurse (RN) license in the state of Maryland.
- Minimum of two years of nursing experience.
- Prior case management experience is advantageous, yet not mandatory.
- Willingness to travel within Baltimore County, Harford County, and Baltimore City for client assessments and visits.
- Strong communication skills, both written and verbal.
