Job Overview

We are seeking a dedicated and compassionate Registered Nurse to join our esteemed Home Health team as a Case Manager. Key Responsibilities

• Conduct comprehensive assessments of patients’ physical, functional, psychosocial, and cognitive health to inform tailored care plans.
• Collaborate with patients, families, and healthcare providers to develop and implement individualized care strategies.
• Continuously monitor and evaluate the effectiveness of care plans, adjusting as necessary to optimize patient results.
• Provide education and support to patients, families, and caregivers regarding safe and effective care practices.
• Work alongside an interdisciplinary team to ensure seamless care coordination and address complex patient requirements.
• Maintain precise and complete patient records in compliance with regulatory and organizational standards.
• Identify and pursue opportunities for quality improvement initiatives to enhance patient care.
  
  

Required Skills

• Active RN licensure in the state.
• Experience with IV therapy and knowledge of venous access devices.
• Strong interpersonal communication skills, with the ability to educate patients and families on self-care.
• Capability to work independently as well as collaboratively within a team setting.
• Familiarity with home health regulations and third-party reimbursement processes.
  
  

Qualifications

• Bachelor's degree in Nursing is preferred.
• At least one year of relevant clinical experience, ideally in a home health or Medicare-certified agency.
• CPR certification is required.
