In this remote role, you will collaborate with primary care physicians to efficiently manage patient panels through the innovative Phamily platform, enhancing the care journey for individuals.
Key Responsibilities
- Collaborate with primary care teams in managing chronic disease patients utilizing the Phamily platform.
- Deliver outstanding customer service through respectful telephone and digital communication, ensuring patients are directed to the appropriate clinical staff when necessary.
- Organize and maintain patient data to facilitate timely and accurate care delivery.
- Identify and address patients' unmet needs while enhancing communication between patients and their healthcare teams.
- Support ongoing quality improvement initiatives, contributing to the redesign of care processes.
- Work collaboratively as a team member, sharing data and insights to optimize patient care.
- Ensure adherence to all relevant healthcare policies, regulations, and procedures.
- Manage a caseload of approximately 500 patients, aiming for 300 billable interactions each month.
- Provide data and insights to care teams to assist in improving patient outcomes.
- Strong communication abilities to effectively engage and inform patients and healthcare teams.
- Self-driven, adaptable, and capable of working both independently and collaboratively.
- Excellent interpersonal and organizational skills.
- A minimum of 2 years of experience in a related field, preferably in an outpatient clinic environment, with direct patient interaction as a caregiver, educator, or advocate.
- Certification as a Medical Assistant (CMA) is preferred, or equivalent experience.
- Current Basic Life Support (BLS) certification.
