Key Responsibilities
- Collaborate with clients, healthcare providers, and members to enhance overall well-being and tackle challenges in their healthcare journey.
- Conduct telephonic outreach, perform data analysis, and undertake clinical reviews while ensuring compliance with relevant federal and state regulations.
- Manage case assignments, including outreach, documentation, monitoring progress, and closing cases to ensure effective member-focused care.
- Assist members in overcoming obstacles to care by addressing social determinants of health and psychosocial issues.
- Provide education to members regarding HMO processes and empower them to make informed healthcare decisions.
- Participate in collaborative meetings with staff and clients, maintaining accurate reporting and documentation.
- Contribute to Quality Management and Utilization Management Committee meetings, including preparation of materials and follow-ups.
- Maintain confidentiality regarding medical records and sensitive program details while adhering to security protocols.
- Active Illinois LCSW, LCPC, or LPC license.
- A minimum of five years of experience in healthcare, managed care, or related settings.
- In-depth understanding of case management principles and healthcare management.
- Proficient in motivational interviewing with strong clinical judgment capabilities.
- Exceptional verbal and written communication skills.
- Strong organizational, problem-solving, and time management abilities.
- Capacity to work independently and remotely in a fast-paced environment while managing multiple tasks.
- Proficiency in Microsoft Office Suite and navigation of multiple Electronic Medical Record (EMR) systems.
- Candidates must possess an active Illinois LCSW, LCPC, or LPC license.
- Five years of relevant experience in healthcare environments or managed care organizations is required.

- Salary: $70,000 annually.
