Key Responsibilities
- Deliver Medication Therapy Management (MTM) services, including in-depth Comprehensive Medication Reviews and Disease-Specific Medication Reviews to evaluate prescribed medications and identify opportunities for improved health outcomes.
- Analyze patients’ medication profiles against evidence-based guidelines to propose recommendations that enhance medication regimens.
- Develop and implement cost-effective strategies for managing medication expenses, including exploring formulary alternatives and identifying unnecessary or duplicate prescriptions.
- Assess prescribed medications for potential issues, such as ineffective therapies, incorrect dosages, adverse drug reactions, and adherence challenges.
- Create and communicate personalized Medication Action Plans (MAPs) and a Personal Medication List (PML), working collaboratively with healthcare providers to ensure optimal patient care.
- Act as a resource for patients, healthcare providers, and interdisciplinary teams, offering expert guidance on medications and therapy options.
- Advocate for MTM services, highlighting their benefits in managing health outcomes to patients, families, and providers.
- Provide exceptional customer service by responding promptly and professionally to inquiries from patients and healthcare providers.
- Follow up diligently with patients and providers to monitor progress and ensure the achievement of treatment objectives.
- Participate in the development and implementation of additional clinical pharmacy services as necessary.
- Meet productivity and performance targets as established by management.
- Strong interpersonal and customer service abilities, ensuring effective communication both in writing and verbally.
- Proficiency with IT tools, including MS Office (Excel, Word) and Google G-Suite.
- A Bachelor's degree in Pharmacy or a Doctor of Pharmacy (Pharm D.) from an accredited institution.
- Minimum of 2 years of experience in a clinical pharmacist role.
- Experience in Medication Therapy Management (MTM) is preferred.
- Active, unrestricted pharmacist licensure in your state, with a willingness to obtain licensure in additional states if required.
