- Manage a patient census while addressing both clinical and non-clinical needs to facilitate improved patient care.
- Connect patients with community resources that support their health and enhance their overall well-being.
- Educate patients on their health conditions, fostering improved health literacy and self-management skills.
- Assist in medication management, including reconciliation, adherence, and refills to promote safe medication practices.
- Coordinate essential services such as home health care, durable medical equipment (DME), and home infusion therapy.
- Engage effectively with patients, answering inquiries and building trust to enhance the patient care experience.
- Support patients in scheduling appointments and managing referrals as necessary.
- Active Multi-State/Compact LPN/LVN License
- Excellent written and verbal communication skills, capable of articulating complex medical information understandably.
- Proficient in Microsoft Office Suite and electronic health records (EHR).
- Completion of an accredited LPN/LVN program.
- Familiarity with Epic EHR is advantageous but not mandatory.
- Bilingual abilities, especially in Spanish, are preferred but not essential.
- Additional single-state LPN licenses are viewed as a plus.
