Key Responsibilities
- Oversee a patient roster, identifying and coordinating both clinical and non-clinical resources to bridge care gaps.
- Connect patients with community resources aimed at enhancing their overall well-being.
- Provide educational support to help patients comprehend their conditions and improve health literacy.
- Assist with medication management, ensuring proper reconciliation, adherence, and timely refills.
- Facilitate access to critical services such as home health care, durable medical equipment (DME), and home infusion needs.
- Foster strong relationships with patients through effective engagement and support.
- Aid patients in scheduling necessary appointments and referrals.
- Active Multi-State/Compact License (LPN/LVN).
- Exceptional communication abilities, both written and verbal, with a focus on simplifying complex medical information for patients.
- Proficiency in technology, including Microsoft Office Suite and electronic health records (EHR).
- Active Multi-State/Compact License (LPN/LVN) is required.
- Previous nursing experience is preferred.
- Experience with Epic EHR is advantageous but not mandatory.
- Bilingual skills (Spanish preferred) are considered a plus.
