Key Responsibilities
- Oversee a patient census with a focus on resolving gaps in clinical and non-clinical care.
- Identify and facilitate access to community resources that could enhance patients’ health management.
- Educate patients to boost health literacy and support chronic condition management.
- Conduct medication management, including reviewing concerns, reconciliation, adherence support, and coordinating refills.
- Ensure prompt delivery of services such as Home Health, Durable Medical Equipment (DME), Home Infusion, and other critical patient needs.
- Foster open communication with patients to understand their requirements, address inquiries, and provide compassionate support.
- Assist in referrals and appointment scheduling to ensure seamless patient care coordination.
- Active Multi-State/Compact License (LPN) (NLC) (LVN).
- Proficient technical skills, particularly in Microsoft Office Suite.
- Strong written and verbal communication capabilities.
- Ability to remain seated for extended periods while using a computer.
- Excellent verbal communication skills for effective telephone-based patient interactions.
- Capability to take detailed notes in real-time during patient calls.
