Key Responsibilities
- Program Leadership: Spearhead our community engagement initiatives by fostering trust and promoting health through a well-structured curriculum. Cultivate a sense of community where members actively engage and support each other.
- Event Planning: Strategically organize and host both virtual and in-person events to strengthen relationships and enhance wellness outcomes. This includes selecting venues, preparing resources, and reaching out to members.
- Member Engagement: Actively reach out to members to encourage participation in community activities. Conduct one-on-one home visits and maintain regular engagement through monthly meetings.
- Documentation: Accurately track all member interactions, including outreach, attendance, and follow-ups using our digital systems for effective monitoring and reporting.
- Leader Identification: Recognize and invite natural leaders within the community to act as ambassadors, enriching member connections and event planning.
- Prospective Member Outreach: Engage potential members, developing individualized plans to enroll them in our programs utilizing diverse engagement techniques.
- Liaison and Partnerships: Facilitate connections between members, health plans, and healthcare services to bridge care gaps effectively.
- Accountability: Ensure accountability for achieving engagement and growth targets, utilizing metrics to gauge attendance and feedback.
- Additional Responsibilities: Undertake other duties as required based on location and company needs, which may include home canvassing.
- Ability to collaborate effectively within a team and foster a positive working environment.
- Proactive approach to community engagement with minimal supervision.
- Adaptability to thrive in a dynamic, fast-paced setting.
- Strong organizational skills to prioritize tasks and meet commitments.
- Proficiency with web-based and app-based systems.
- Capacity to work independently in a remote environment.
- Reliable transportation for community visits and outreach.
- Flexible availability to accommodate events outside regular business hours.
- Minimum of 3 years of experience in community outreach, teaching, or facilitation; healthcare experience is preferred.
- Proven experience in building or supporting a community of 500 members.
- An associate’s degree is preferred; however, relevant experience may be accepted in lieu of education.
- Willingness to complete pre-employment drug testing and background checks.
