This full-time, remote position offers you the flexibility to provide essential healthcare support while contributing to your local community.
Key Responsibilities
- Develop and execute tailored care plans that cater to the physical, behavioral, social, and long-term needs of our members.
- Conduct both telephonic and in-person clinical assessments to identify, evaluate, and address member needs effectively.
- Coordinate care for high-risk members facing chronic conditions, co-morbidities, or disabilities.
- Optimize care management to ensure efficient utilization of health benefits, including addressing gaps in care.
- Facilitate authorizations, referrals, and the implementation of care plans.
- Collaborate with Medical Directors, Physician Advisors, and interdisciplinary teams to enhance treatment planning.
- Maintain compliance with relevant regulations, accreditation standards, and internal policies.
- Provide solutions for issues related to providers, claims, or services.
- Active Registered Nurse (RN) license - Compact/Multi-State license required.
- Preferred: Bachelor of Science in Nursing (BSN).
- Relevant nursing experience emphasizing case management, care coordination, or patient advocacy.
- Strong insight into the physical, psychological, socio-cultural, and cognitive aspects of patient care.
- Exceptional communication (both oral and written), analytical, and problem-solving abilities.
- Capacity to work autonomously and collaboratively in a dynamic, remote setting.
- For High Intensity Care Management, minimum one year of experience working directly with members who:
- Require private duty nursing services;
- Are life-sustaining ventilator dependent; or
- Are vulnerable infants (e.g., NAS, substance-exposed, NICU level 3).
- Nursing experience focusing on case management, care coordination, or patient advocacy is essential.
