We are seeking a dedicated and compassionate Transition Therapist to join our remote team. This pivotal role is focused on empowering young individuals as they navigate significant life transitions, ultimately making a profound impact on their mental health and well-being.
Key Responsibilities
- Provide comprehensive assessments, care, and support planning for youth and young adults (ages 16-25) experiencing serious mental illnesses and/or co-occurring disabilities.
- Assist in the development and execution of youth-driven transition plans, facilitating their successful movement from child to adult systems.
- Ensure services adhere to a System of Care approach, prioritizing youth voice and utilizing trauma-informed methodologies.
- Collaborate within a multi-disciplinary team and cultivate strong relationships with various partners, both internal and external.
- Contribute to the success of programs by supporting grant objectives, engaging in outreach initiatives, and aiding in the training of new team members.
- Provide clinical expertise by conducting screenings, assessments, and care plan development, while ensuring timely documentation.
- Participate in community engagement events and training, collaborating with stakeholders who support transition-age youth.
- Remain informed about industry trends and best practices relevant to the support of transition-age youth.
- Strong commitment to System of Care principles, trauma-informed care, and person-centered approaches.
- Excellent communication and interpersonal skills, with a proven ability to work collaboratively in a team environment.
- Ability to operate independently and effectively manage time.
- Master’s Degree in a relevant human services field.
- Full or associate-level clinical board licensure in North Carolina (LCSW, LCSW-A, LCMHC, LCMHC-A, LMFT, or LMFT-A).
- A minimum of one year of experience working with youth and young adults (ages 16-25).
- A valid NC driver’s license and insurance are required.

- Competitive salary range: \(50,000 - \)60,000 per year.
