Key Responsibilities
- Conduct remote therapy sessions, perform diagnostic assessments, and develop care plans in collaboration with clients and their families.
- Deliver individual, family, or group therapy as necessary, ensuring high-quality and compassionate care.
- Engage in care coordination, working closely with both internal and external providers to discuss treatment plans and monitor client progress.
- Document all clinical interventions in accordance with state and regulatory standards, ensuring compliance and accuracy.
- Participate in clinical team meetings, contributing insights regarding client care and treatment efficacy.
- Maintain productivity standards while promoting a safe and supportive environment for clients to express their emotions and build resilience.
- Perform crisis interventions and evaluate safety concerns, including risks related to self-harm and domestic violence.
- Guide clients in developing effective coping strategies and personal goal attainment.
- Create, update, and evaluate treatment plans to reflect the evolving needs of clients, ensuring they meet clinical standards.
- Identify and leverage community mental health resources to enrich client therapy and overall well-being.
- Valid CT LCSW, LPC, or LMFT license issued by the Department of Public Health (DPH).
- Proficient in conducting behavioral health evaluations and providing therapy to children and adults.
- Strong collaborative skills with clinical teams and the ability to perform crisis interventions effectively.
- Excellent communication skills with an understanding of community mental health resources.
- Master’s degree in Social Work, Counseling, or a related discipline.
- Experience in providing behavioral health therapy in an outpatient setting is preferred.
