Key Responsibilities
- Conduct regular meetings with assigned cases and stakeholders to evaluate needs and formulate personalized support plans.
- Provide crisis intervention, counseling, and problem-solving assistance to program participants in areas such as mental health, housing, and employment.
- Address psychosocial challenges and offer guidance to enhance engagement and self-sufficiency.
- Collaborate with participants to develop, implement, and monitor individualized service plans.
- Advocate for participants to access benefits such as income support and entitlement programs.
- Oversee treatment plans and ensure that goals are achieved within established timelines.
- Offer financial assistance and support in budget management as applicable.
- Complete case documentation and reports accurately and in a timely manner.
- Maintain an appropriate work volume while employing strengths-based clinical techniques to meet participants' needs.
- Excellent communication and relationship-building abilities.
- Capacity to collaborate effectively with diverse teams and stakeholders.
- Experience in managing confidential information with utmost professionalism.
- Proficiency in case management software and Microsoft Office Suite.
- Strong problem-solving capabilities and adaptability to changing situations.
- Education: Bachelor's degree in Social Work, Psychology, or a related field, coupled with three years of experience in case management or counseling; or a Master's degree in a related field with one year of relevant experience.
- Licensure: Must possess a valid Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) certification.
