Key Responsibilities
- Conduct regular meetings with assigned cases and stakeholders to evaluate needs and develop tailored support plans.
- Provide crisis intervention, counseling, and solutions to program participants in critical areas such as mental health, housing, and employment.
- Address psychosocial challenges and deliver guidance to improve participant engagement and self-sufficiency.
- Create, implement, and monitor customized service plans in collaboration with participants.
- Screen participants and advocate for their access to benefits, including income support and entitlement programs.
- Manage treatment plans and ensure objectives are met within established timelines.
- Provide financial assistance and budget management support as needed.
- Complete case documentation and reports promptly and accurately.
- Maintain an appropriate workload and apply strengths-based clinical techniques to address participants’ needs.
- Exceptional communication and relationship-building capabilities.
- Proven ability to work effectively within diverse teams and with various stakeholders.
- Experience handling confidential information with professionalism and discretion.
- Proficiency in case management software and the Microsoft Office Suite.
- Strong problem-solving skills with adaptability to new circumstances.
- Education: A Bachelor's degree in Social Work, Psychology, or a related field, accompanied by three years of case management or counseling experience, or a Master's degree in a related field with one year of relevant experience.
- Licensure: Must possess a valid Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) certification.
