Key Responsibilities
- Regularly meet with assigned cases and relevant stakeholders to assess needs and develop personalized support strategies.
- Provide crisis intervention, counseling, and problem-solving assistance to program participants concerning mental health, housing, and employment.
- Address psychosocial issues and provide guidance to enhance engagement and self-sufficiency.
- Create, implement, and monitor individualized service plans in collaboration with participants.
- Advocate for participants to access benefits such as income support and entitlement programs.
- Supervise treatment plans and ensure objectives are achieved within designated timelines.
- Offer financial assistance and budget management support as applicable.
- Ensure timely and accurate completion of case documentation and reports.
- Maintain case load and employ strengths-based clinical techniques to address participants' needs.
- A Bachelor's degree in Social Work, Psychology, or a related discipline, coupled with three years of experience in case management or counseling; or a Master's degree in a related field with one year of relevant experience.
- Must possess a valid Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) certification.
- Excellent communication and relationship-building abilities.
- Capability to work effectively with diverse teams and stakeholders.
- Experience managing confidential information with professionalism and discretion.
- Proficiency in case management software and Microsoft Office Suite.
- Strong problem-solving skills with the capacity to adapt to changing circumstances.
