Key Responsibilities
- Facilitate medical management and health education initiatives for participants in government healthcare programs.
- Collect and evaluate data to generate regulatory compliance reports.
- Conduct assessments and manage care coordination through telephonic and in-person engagements with members and their families.
- Craft personalized care plans that cater to the distinct needs of individuals.
- Represent the organization with professionalism in all interactions with members, healthcare providers, and community resources.
- Travel within your designated territory as required.
- Proven experience as a Registered Nurse (RN) with a minimum of 3 years of direct clinical care, or as a Licensed Professional Counselor (LPC) or Licensed Master Social Worker (LMSW) with at least 2 years of clinical experience.
- Exceptional verbal and written communication abilities, facilitating discussions around medical needs with members and collaboration with internal and external teams.
- Strong analytical skills demonstrated by experience in reviewing medical data.
- Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint, as well as web-based applications.
- Willingness and capability to travel within the assigned region.
- A current, valid, and unrestricted license in the state of practice (or eligibility for reciprocity).
- Minimum of 3 years of experience in wellness or managed care, including presenting clinical matters to members or physicians.
- Comprehensive knowledge of the health and wellness sector, along with familiarity with trends in healthcare offerings.
