Key Responsibilities
- Conduct comprehensive psychosocial assessments to identify clients’ mental health requirements.
- Collaborate with healthcare teams to design and execute personalized treatment plans.
- Establish and nurture strong therapeutic relationships with patients, families, and guardians, facilitating access to vital resources that encourage recovery.
- Provide individual, family, and group therapy, employing a variety of recognized therapeutic techniques.
- Work alongside the treatment team to manage discharge planning and address significant issues affecting client care.
- Advocate for clients to secure necessary resources and support.
- Coordinate with other healthcare providers to ensure holistic and integrated care.
- Deliver crisis intervention services during psychiatric emergencies.
- Educate clients and their families on mental health conditions, possible treatment options, and available resources.
- Maintain thorough and confidential records of client interactions.
- Facilitate access to community services and support networks.
- Monitor client progress, adapting treatment plans as required.
- Oversee or assist with patient transportation needs when necessary.
- Initiate or manage guardianship and involuntary commitment processes, ensuring adherence to ethical standards.
- Support colleagues in delivering exemplary and clinically sound patient interactions.
- Provide clinical insights and recommendations during diagnostic and treatment review conferences.
- Assist with emergent social service needs as they arise.
- Minimum of one year of clinical experience in direct patient care as a Licensed Clinical Social Worker.
- Experience across diverse settings such as detention facilities, residential healthcare, hospitals, outpatient clinics, and nursing homes.
- Proficient in working with both pediatric and adult populations.
- Mastery of therapeutic techniques and strong interpersonal skills.
- Basic Life Support (BLS) Certification from an accredited organization.
- Effective verbal and written communication skills in English.
- Ability to travel on short notice and provide services across different medical units.
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current licensure in New Hampshire with no history of disciplinary actions.
- U.S. Citizenship or Permanent Resident status is required.
- Reliable access to Concord, NH, or willingness to relocate is essential.
