Key Responsibilities

• Conduct comprehensive psychosocial assessments to identify clients’ mental health needs.
• Collaborate with healthcare teams to develop and implement individualized treatment plans.
• Establish and maintain therapeutic relationships with patients, families, and other stakeholders to facilitate access to necessary resources.
• Deliver individual, family, and group therapy while employing a variety of therapeutic techniques.
• Participate in discharge planning discussions with the treatment team and relevant parties.
• Advocate for clients to ensure they receive suitable resources and support.
• Coordinate with other healthcare professionals to provide holistic care.
• Offer crisis intervention services during psychiatric emergencies.
• Educate clients and their families about mental health conditions and available treatment options.
• Maintain accurate and confidential client records in compliance with applicable regulations.
• Facilitate access to community resources and support services.
• Monitor and evaluate clients’ progress, adjusting treatment plans as necessary.
• Oversee transportation services for patients to medical appointments.
• Initiate guardianship or involuntary commitment procedures when required, ensuring adherence to ethical standards.
• Support colleagues in reinforcing courteous interactions and appropriate interventions.
  
  

Required Skills

• Minimum of one year of clinical experience in direct patient care as a licensed Clinical Social Worker.
• Experience across diverse healthcare environments including detention, correctional, residential, and outpatient settings.
• Proficient in working with both pediatric and adult populations.
• Current license to practice in New Hampshire, free from disciplinary actions.
• Master’s degree in Social Work (MSW) from an accredited institution.
• Valid BLS certification from a recognized organization.
• Strong communication skills in English, both verbal and written.
• U.S. Citizenship or Permanent Resident status.
• Willingness to commute or relocate to Concord, NH, and travel on short notice as needed.
