Key Responsibilities
- Conduct comprehensive psychosocial assessments to identify clients’ mental health needs.
- Collaborate with healthcare teams to develop and implement individualized treatment plans.
- Establish and maintain therapeutic relationships with patients, families, and other stakeholders to facilitate access to necessary resources.
- Deliver individual, family, and group therapy while employing a variety of therapeutic techniques.
- Participate in discharge planning discussions with the treatment team and relevant parties.
- Advocate for clients to ensure they receive suitable resources and support.
- Coordinate with other healthcare professionals to provide holistic care.
- Offer crisis intervention services during psychiatric emergencies.
- Educate clients and their families about mental health conditions and available treatment options.
- Maintain accurate and confidential client records in compliance with applicable regulations.
- Facilitate access to community resources and support services.
- Monitor and evaluate clients’ progress, adjusting treatment plans as necessary.
- Oversee transportation services for patients to medical appointments.
- Initiate guardianship or involuntary commitment procedures when required, ensuring adherence to ethical standards.
- Support colleagues in reinforcing courteous interactions and appropriate interventions.
- Minimum of one year of clinical experience in direct patient care as a licensed Clinical Social Worker.
- Experience across diverse healthcare environments including detention, correctional, residential, and outpatient settings.
- Proficient in working with both pediatric and adult populations.
- Current license to practice in New Hampshire, free from disciplinary actions.
- Master’s degree in Social Work (MSW) from an accredited institution.
- Valid BLS certification from a recognized organization.
- Strong communication skills in English, both verbal and written.
- U.S. Citizenship or Permanent Resident status.
- Willingness to commute or relocate to Concord, NH, and travel on short notice as needed.
- A solid foundation in clinical practice with a focus on mental health and substance abuse challenges.
- Ability to navigate complex legal issues related to patient care and rights.
