We invite you to join our esteemed organization as an Individual Risk Assessment/Reduction Case Manager, a role dedicated to making a profound impact in the lives of older adults. This full-time position offers you the flexibility of remote work, complemented by occasional travel within Pennsylvania.

Key Responsibilities

  • Conduct comprehensive evaluations of protective services cases through meticulous investigative efforts.
  • Engage with diverse collateral contacts, including family members, alleged perpetrators, healthcare professionals, and financial institutions to gather pertinent case information.
  • Ensure adherence to protective services legislation and operational protocols while respecting statutory timelines.
  • Evaluate potential risks to older adults and identify any unmet needs necessitating further intervention.
  • If risks or needs are confirmed, refer the case to the appropriate agency and develop a tailored service plan.
  • If no further action is warranted, responsibly close the case in alignment with established guidelines.
  • Collaborate with agency-based medical professionals to facilitate case-related assessments as required.
  • Maintain precise documentation of all case-related activities, including interviews and findings, and input all relevant data into the mandated state system.

Required Skills

  • Strong investigative and analytical abilities.
  • Excellent written and verbal communication skills.
  • Capacity to evaluate risks and discern unmet needs.
  • Knowledge of protective services legislation and operational procedures.
  • Proficient in documentation and case management practices.

Qualifications

  • A Bachelor's degree in a related field.
  • A minimum of two years of professional experience in social services or a similar human services program.
  • Essential experience in Individual Risk Assessment and Reduction.
  • Preferred: At least one year of direct involvement in aging-related casework or a combination of relevant training and experience.

Compensation And Benefits
