Key Responsibilities
- Conduct outreach to members with identified gaps in preventive care.
- Educate members regarding the significance of preventive care and necessary screenings.
- Follow up with members requiring assistance to address care gaps.
- Assess health-related social determinants impacting service accessibility.
- Evaluate ongoing care management requirements and facilitate appropriate referrals.
- Actively participate in Quality Workgroups focusing on critical performance measures and quality initiatives.
- Maintain accurate documentation of member interactions in the clinical system.
- Generate reports to interpret specifications, elucidate programs and results, and document findings.
- Uphold quality and timeliness standards in individual and departmental objectives.
- Stay updated on necessary systems, procedures, forms, and manuals.
- Adhere to all pertinent state and federal regulations.
- Organize quality improvement meetings and discussions.
- Identify and escalate any process gaps needing remedial action.
- Develop and uphold policies and standard operating procedures to guarantee compliance.
- Support clinical best practices aimed at enhancing health outcomes, preventing readmissions, ensuring member safety, and advocating for wellness.
- Execute additional duties as assigned.
- Associate's degree or nursing diploma; Bachelor's or Master's degree preferred.
- Minimum of 2 years of experience as a practicing RN in a hospital or healthcare environment; health insurance experience preferred.
- Active, current, and unrestricted RN license in Massachusetts.
- Proficient in office equipment and software (e.g., word processing, spreadsheets, databases).
- Strong collaborative and process improvement abilities.
- Exceptional oral and written communication skills.

- Understanding of contemporary healthcare trends.
