Key Responsibilities
- Perform comprehensive assessments and wellness examinations, both in-home and virtually.
- Educate and counsel patients and their families regarding available benefits and healthcare programs.
- Document detailed patient information, including medical history, assessments, and treatment plans.
- Collaborate in coordinating care with primary care physicians, specialists, and other healthcare providers.
- Maintain high standards of care and deliver evidence-based practices that enhance patient satisfaction and health outcomes.
- Facilitate education on preventive care and assist patients in accessing essential resources.
- Engage actively in team meetings to discuss patient care and treatment strategies.
- Develop and implement effective treatment plans while monitoring patient progress.
- Bilingual proficiency in English and Spanish is highly preferred.
- Active and unrestricted licensure as a Registered Nurse and Nurse Practitioner or Physician Assistant.
- Strong communication skills, both verbal and written.
- Familiarity with data management systems and medical information technologies.
- Comprehensive knowledge of federal and state healthcare regulations, including HIPAA and OSHA.
- Minimum of 2 years' experience in direct patient care.
- Experience in Managed Care, IPA, or Health Plan settings is advantageous.
- Board certification (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA license.
- Proven ability to work autonomously and make independent clinical decisions.
- Valid driver's license and access to reliable transportation.
