Key Responsibilities
- Conduct in-depth assessments, including annual wellness exams, for patients in their homes and via virtual platforms.
- Counsel and educate patients and their families about health management programs and available benefits.
- Maintain accurate and thorough documentation of patient information, including medical history and treatment plans.
- Collaborate with primary care providers, specialists, and ancillary services to ensure comprehensive care.
- Deliver evidence-based care aimed at enhancing patient health and minimizing avoidable hospitalizations.
- Monitor and evaluate patient outcomes, continually working to improve health quality indicators.
- Participate in integrated care team meetings for collaborative treatment planning.
- Perform additional responsibilities as required.
- Bilingual proficiency (English/Spanish) is highly preferred.
- Basic Life Support (BLS) certification (assistance with obtaining can be provided).
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification relevant to licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Proficient in using medical instruments and equipment, along with data management systems.
- Strong verbal and written communication skills.
- Familiarity with federal and state regulations, including HIPAA and OSHA.
- Minimum of 2 years of direct patient care experience, preferably in managed care settings.
- Successful completion of an accredited Nursing or Physician Assistant program.
- Proven track record in conducting annual wellness visits or similar comprehensive evaluations.
