Job Overview

We are seeking a dedicated and empathetic Nurse Practitioner or Physician Assistant to join our pioneering in-home care team in Sierra Vista, AZ. Key Responsibilities

• Conduct in-depth assessments, including annual wellness exams, for patients in their homes and via virtual platforms.
• Counsel and educate patients and their families about health management programs and available benefits.
• Maintain accurate and thorough documentation of patient information, including medical history and treatment plans.
• Collaborate with primary care providers, specialists, and ancillary services to ensure comprehensive care.
• Deliver evidence-based care aimed at enhancing patient health and minimizing avoidable hospitalizations.
• Monitor and evaluate patient outcomes, continually working to improve health quality indicators.
• Participate in integrated care team meetings for collaborative treatment planning.
• Perform additional responsibilities as required.
  
  

Required Skills

• Bilingual proficiency (English/Spanish) is highly preferred.
• Basic Life Support (BLS) certification (assistance with obtaining can be provided).
• Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
• Board certification relevant to licensure (NP: ANCC/AANP; PA: NCCPA).
• Current and unrestricted DEA certificate.
• Proficient in using medical instruments and equipment, along with data management systems.
• Strong verbal and written communication skills.
• Familiarity with federal and state regulations, including HIPAA and OSHA.
• Minimum of 2 years of direct patient care experience, preferably in managed care settings.

• Successful completion of an accredited Nursing or Physician Assistant program.
• Proven track record in conducting annual wellness visits or similar comprehensive evaluations.
