Key Responsibilities
- Conduct thorough assessments, including annual wellness exams, in both virtual and in-home settings.
- Provide counseling and education to patients and their families regarding benefits and programs that promote healthier lifestyles.
- Maintain precise and up-to-date patient records, ensuring all relevant information is documented accurately.
- Collaborate effectively with primary care providers, specialists, and ancillary services to coordinate comprehensive care.
- Complete necessary documentation and paperwork promptly.
- Deliver timely, evidence-based care aimed at reducing avoidable hospitalizations while prioritizing patient health and satisfaction.
- Prescribe medications, order tests, and work closely with physicians involved in patient care.
- Facilitate education regarding preventive monitoring and assist patients in scheduling diagnostic testing.
- Actively participate in integrated care team meetings to enhance collaboration.
- Develop and implement effective treatment plans tailored to individual patient needs.
- Follow up and assess patient health status regularly.
- Perform additional duties as required.
- Active and unrestricted Nurse Practitioner or Physician Assistant license alongside a Registered Nurse license.
- Board certification relevant to the licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certification.
- Exceptional ability to work autonomously and independently.
- Proficiency in utilizing medical instruments, equipment, and medical records systems.
- Strong verbal and written communication capabilities.
- Familiarity with federal and state regulations, including HIPAA and OSHA guidelines.
- A minimum of 2 years of direct patient care experience; Managed Care/IPA/Health Plan experience is preferred.
- Previous experience with wellness visits or similar comprehensive assessments is advantageous.
- Bilingual in English and Spanish is highly desirable.
- Basic Life Support (BLS) certification is essential, with support available for certification completion.
- A relevant degree in nursing or physician assistant studies.
- At least 2 years of pertinent experience in direct patient care.
