Key Responsibilities
- Conduct thorough assessments, including annual wellness exams, for patients either virtually or in their homes.
- Offer guidance and education to patients and their families regarding available resources and health programs.
- Maintain up-to-date and accurate patient records documenting all relevant medical information.
- Collaborate with primary care providers, specialists, and ancillary services to coordinate patient care effectively.
- Deliver evidence-based care focused on enhancing quality of life and minimizing hospitalizations.
- Prescribe medications, manage testing, and work closely with physicians to ensure comprehensive patient care.
- Educate patients on preventive monitoring and facilitate access to educational resources.
- Participate in integrated care team meetings to optimize patient outcomes.
- Active and unrestricted Registered Nurse and Nurse Practitioner license.
- Board certification relevant to licensure (NP: ANCC/AANP).
- Current and unrestricted DEA certificate.
- Strong communication skills, both verbal and written.
- Proficiency with medical instruments and data management systems.
- Solid understanding of federal and state regulations, including HIPAA and OSHA.
- Bilingual proficiency (English/Spanish) is highly desirable.
- A minimum of 2 years of direct patient care experience is required.
- Experience with Managed Care/IPA/Health Plans is preferred.
- Demonstrated experience conducting annual wellness visits or similar assessments.
