Key Responsibilities
- Perform comprehensive assessments, including annual wellness examinations, in both home and virtual settings.
- Educate and counsel patients and their families regarding available resources to promote healthier living.
- Maintain detailed documentation of patient interactions, including complaints, diagnoses, medical history, and treatment plans.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Ensure prompt completion of all documentation and adherence to high-quality care standards.
- Deliver evidence-based care to minimize avoidable hospitalizations and enhance patient satisfaction.
- Prescribe medications and collaborate with healthcare professionals to optimize patient outcomes.
- Facilitate patient education and support in accessing essential resources.
- Actively participate in integrated care team meetings.

- Active and unrestricted licenses as a Registered Nurse and Nurse Practitioner.
- Board certification and a current DEA certificate.
- Proven ability to work independently while practicing autonomously.
- Proficiency in medical instruments, electronic data management, and point-of-care technology.
- Strong verbal and written communication capabilities.
- In-depth knowledge of federal and state healthcare regulations, including HIPAA and OSHA.
- Minimum of two years of direct patient care experience, with a background in managed care or similar settings.
- Experience conducting wellness visits or comprehensive assessments in-home or virtually.
- Active and unrestricted licenses as a Registered Nurse and Nurse Practitioner obtained from an accredited program.
- Minimum of two years of experience in direct patient care, with a preference for experience in chronic illness management and health education.
