Key Responsibilities
- Conduct comprehensive assessments, including annual wellness exams, both virtually and in patients’ homes.
- Educate and counsel patients and their families regarding available resources and programs aimed at promoting healthier living.
- Accurately document patient information, encompassing chief complaints, diagnoses, medical history, examinations, medications, and treatment plans.
- Collaborate with primary care providers, specialists, and ancillary services to coordinate optimal patient care.
- Complete necessary documentation with efficiency and maintain high standards of care quality.
- Deliver evidence-based and timely care, aiming to minimize avoidable hospitalizations while enhancing the quality of life for patients.
- Prescribe medications, order laboratory tests, and collaborate closely with patients’ physicians.
- Facilitate the closure of care gaps by educating patients about preventive monitoring and routine diagnostic testing.
- Assist patients in accessing relevant educational materials to support their health journeys.
- Participate in integrated care team meetings and contribute to the assessment and implementation of treatment plans.
- Follow up on patient progress and adjust treatment plans as needed.
- Other duties as assigned.
- An active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification relevant to the licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Proficiency with medical instruments and advanced medical equipment.
- Familiarity with computer-based data management programs and medical records systems.
- Excellent verbal and written communication skills.
- Comprehensive understanding of federal and state regulations, including HIPAA and OSHA compliance.
- Bilingual proficiency in English and Spanish is highly preferred.
- A minimum of 2 years of direct patient care experience.
- Additional experience in Managed Care, IPA, or Health Plans is advantageous.
- Experience conducting annual wellness visits or similar comprehensive assessments, either virtually or in-home, is a plus.
