Job Overview

We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our innovative care team, offering an exceptional opportunity to make a meaningful impact in the lives of patients with complex health needs, particularly those affected by chronic kidney disease and end-stage renal disease. This role allows you to provide personalized, in-home care, contributing to the well-being of our patients.

Key Responsibilities

  • Conduct comprehensive assessments, including annual wellness exams, both virtually and in patients’ homes.
  • Educate and counsel patients and their families regarding available resources and programs aimed at promoting healthier living.
  • Accurately document patient information, encompassing chief complaints, diagnoses, medical history, examinations, medications, and treatment plans.
  • Collaborate with primary care providers, specialists, and ancillary services to coordinate optimal patient care.
  • Complete necessary documentation with efficiency and maintain high standards of care quality.
  • Deliver evidence-based and timely care, aiming to minimize avoidable hospitalizations while enhancing the quality of life for patients.
  • Prescribe medications, order laboratory tests, and collaborate closely with patients’ physicians.
  • Facilitate the closure of care gaps by educating patients about preventive monitoring and routine diagnostic testing.
  • Assist patients in accessing relevant educational materials to support their health journeys.
  • Participate in integrated care team meetings and contribute to the assessment and implementation of treatment plans.
  • Follow up on patient progress and adjust treatment plans as needed.
  • Other duties as assigned.

Required Skills

  • An active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
  • Board certification relevant to the licensure (NP: ANCC/AANP; PA: NCCPA).
  • Current and unrestricted DEA certificate.
  • Proficiency with medical instruments and advanced medical equipment.
  • Familiarity with computer-based data management programs and medical records systems.
  • Excellent verbal and written communication skills.
  • Comprehensive understanding of federal and state regulations, including HIPAA and OSHA compliance.
  • Bilingual proficiency in English and Spanish is highly preferred.

Qualifications

  • A minimum of 2 years of direct patient care experience.
  • Additional experience in Managed Care, IPA, or Health Plans is advantageous.
  • Experience conducting annual wellness visits or similar comprehensive assessments, either virtually or in-home, is a plus.
