Key Responsibilities
- Collaborate with an integrated care team, including a Registered Nurse and Social Worker, to deliver personalized medical care primarily in-home and through virtual consultations.
- Conduct thorough assessments, including comprehensive annual wellness exams.
- Educate and counsel patients and their families regarding available benefits and healthcare programs.
- Maintain accurate and timely documentation of patient information and care plans.
- Coordinate effectively with primary care providers, specialists, and ancillary services to ensure seamless care.
- Uphold high standards of care and deliver evidence-based practices that enhance patient satisfaction and health outcomes.
- Proficiency in medical instruments and equipment.
- Strong communication skills, both verbal and written.
- Familiarity with computer-based data management systems.
- Understanding of federal and state healthcare regulations, including HIPAA and OSHA.
- Bilingual proficiency in English and Spanish is highly preferred.
- Active and unrestricted licenses as a Registered Nurse and Nurse Practitioner or Physician Assistant.
- Board certified (NP: ANCC/AANP; PA: NCCPA) and possess a current DEA certificate.
- Minimum of 2 years of direct patient care experience, preferably in a managed care or similar setting.
