Key Responsibilities
- Conduct thorough assessments, including annual wellness examinations, in-home and virtually.
- Educate and counsel patients and their families on available healthcare benefits and resources.
- Maintain precise and updated patient records, documenting all relevant medical information.
- Collaborate with primary care providers, specialists, and ancillary services to coordinate patient care.
- Ensure timely completion of documentation while upholding high standards of care.
- Provide evidence-based medical care aimed at minimizing avoidable hospitalizations and enhancing patient quality of life.
- Prescribe medications, initiate tests, and work closely with patients’ physicians.
- Facilitate patient education on preventive measures and assist in scheduling necessary diagnostic tests.
- Participate actively in integrated care team meetings and evaluations of patients’ health status.
- Develop and implement tailored treatment plans and track patient progress.
- Perform additional duties as assigned.
- Active and unrestricted RN and Nurse Practitioner or Physician Assistant licensure.
- Board certification (NP: ANCC/AANP; PA: NCCPA).
- Current and valid DEA certification.
- Proficiency with medical instruments and familiarity with data management systems.
- Strong communication skills, both verbal and written.
- Knowledge of relevant federal and state regulations (HIPAA, OSHA).
- Bilingual proficiency in English and Spanish is highly preferred.
- A minimum of 2 years of direct patient care experience in a healthcare setting.
- Experience with managed care systems, including IPA and health plans.
- Prior experience conducting comprehensive wellness visits, either virtually or at home.
