Key Responsibilities
- Perform thorough assessments, including annual wellness exams, both in patients’ homes and via virtual consultations.
- Educate and guide patients and their families regarding available health benefits and programs.
- Maintain accurate and detailed patient records, documenting complaints, diagnoses, medical history, examinations, and treatment plans.
- Collaborate effectively with primary care providers, specialists, and other healthcare services to ensure continuity of care.
- Complete all necessary documentation and paperwork in a timely manner while adhering to high standards of care.
- Provide timely, evidence-based medical intervention that enhances patient health and minimizes hospital visits.
- Prescribe medications, order diagnostic tests, and work closely with patients’ healthcare teams.
- Participate in integrated care team meetings and contribute to strategies that drive improved patient outcomes.
- Bilingual (English/Spanish) capabilities are highly desired.
- Possession of an active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification (NP: ANCC/AANP; PA: NCCPA) is required.
- Current and valid DEA certificate.
- Demonstrated ability to work autonomously and independently.
- Strong verbal and written communication skills.
- Proficiency in utilizing medical instruments and equipment.
- Familiarity with computer-based data management systems and medical records.
- Knowledge of federal and state regulations, including HIPAA and OSHA compliance.
- A minimum of two years of direct patient care experience is essential.
- Experience with managed care, IPA, or health plans is preferable.
- Prior experience conducting comprehensive wellness visits (virtual or in-home) is beneficial.
