Key Responsibilities
- Conduct comprehensive assessments and annual wellness exams, both virtually and in patients’ homes.
- Educate and counsel patients and their families on available health benefits and support programs.
- Maintain meticulous documentation of patient records, including medical histories, examinations, and treatment plans.
- Collaborate with healthcare professionals to ensure coordinated and comprehensive patient care.
- Complete documentation in a timely manner while adhering to quality care standards.
- Provide evidence-based care that enhances patients’ quality of life and reduces unnecessary hospitalizations.
- Prescribe medications, order diagnostic tests, and work closely with patients’ primary care physicians.
- Facilitate patient education by providing access to relevant educational resources.
- Participate actively in integrated care team discussions and planning sessions.
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Relevant board certification (NP: ANCC/AANP; PA: NCCPA).
- Current DEA certificate without restrictions.
- Strong verbal and written communication abilities.
- Proficiency in medical instruments and technology for data management.
- Knowledge of federal and state healthcare regulations, including HIPAA and OSHA compliance.
- A minimum of two years of direct patient care experience.
- Experience in managed care or health plan environments is beneficial.
- Bilingual capabilities (English/Spanish) are preferred.
- Master's degree in Nursing or Physician Assistant studies.
- Two or more years of direct patient care experience with a focus on annual wellness visits or comprehensive assessments in home settings or virtually.
