Key Responsibilities
- Deliver personalized care through comprehensive assessments, including annual wellness exams in patient homes and via virtual platforms.
- Empower patients and their families by providing essential education about health benefits and social programs.
- Maintain accurate and thorough documentation, including diagnoses, medical histories, and care plans.
- Collaborate effectively with primary care providers and specialists to ensure holistic patient care.
- Complete documentation in a timely and precise manner.
- Adhere to established quality care standards while delivering evidence-based interventions.
- Prescribe medications, order necessary tests, and engage in collaborative care with patients' physicians.
- Play a role in driving improved patient health outcomes through effective healthcare delivery.
- Educate patients on preventive monitoring and facilitate necessary testing through physician collaborations.

- Valid and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification for relevant licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Proficiency in medical instruments and equipment.
- Strong communication skills, both verbal and written.
- Familiarity with healthcare regulations, including HIPAA and OSHA.
- Two or more years of direct patient care experience.
- Experience in managed care environments and conducting wellness visits.
- Bilingual skills (English/Spanish) are highly desirable.
- Relevant degree in Nursing or Physician Assistant studies.
- Master's degree or higher for Nurse Practitioners or Physician Assistants.
