Key Responsibilities
- Conduct initial and ongoing diagnostic assessments, integrating physical health considerations into treatment.
- Develop collaborative treatment plans alongside primary and specialty care providers.
- Assess eligibility for state-funded services as needed.
- Deliver individual and group therapy in various settings, including telehealth options.
- Support clients in developing and applying coping strategies through counseling.
- Collaborate with families to foster client resilience and recovery through training and counseling.
- Offer psycho-education utilizing Integrated-Illness Management and Recovery (I-IMR) strategies.
- Provide crisis management and intervention as necessary.
- Adhere to the practices established by the Integrated Care Model, collaborating with treatment teams and external providers.
- Maintain client confidentiality and uphold professional ethical standards.
- Document client interactions timely and accurately while participating in consultation meetings and committees.
- Master's degree in a mental health field such as social work, clinical mental health counseling, or psychology.
- Licensed Clinical Mental Health Counselor (LCMHC) or Licensed Independent Clinical Social Worker (LICSW) preferred, or eligibility for licensure.
- Strong understanding of chronic co-morbid physical health conditions and experience with medically complex clients is advantageous.
- Proficient in Microsoft Office applications and electronic health records.
- Excellent documentation, communication, organization, and multitasking skills.
- Ability to work independently while also thriving in a team environment.
- Openness to acquiring new clinical skills.
- Master's degree in a relevant mental health discipline.
- Preferred qualifications include licensure as an LCMHC or LICSW.
