Get Set Go Health & Wellness Company, LLC is seeking well-organized individuals who have a passion for helping clients cope with life challenges to obtain the support they need to achieve their goals. The Targeted Case Manager (TCM) will work with kid and adult clients seeking behavioral health services to identify additional needs and assist them in connecting to other agency and community resources. Successful TCM staff adapt their approach to engaging clients with different backgrounds, styles of commutation, and needs. They will enjoy creative problem solving and an ability to work independently while also actively contributing to their team of coworkers. This is an ideal position for individuals who have strong interpersonal skills that wish to gain additional experience working with clients with mental health and substance use disorders.
NECESSARY SKILLS:
- Ability to work independently and with multiple priorities
- Good communication skills, both spoken and written
- Experience using computerized documentation systems
- Time management skills
- Must have own reliable transportation
- Must be able to work flexible hours to accommodate the needs of individuals and their support network
- Must be able to facilitate meetings of team members/support network for individual
- Good problem-solving skills
Qualifications:
- Minimum of six months experience in the field of developmental disabilities.
- Bachelor’s degree or additional experience in the field substituted for the degree at the rate of six months of experience for each semester.
- Successfully complete all modules of the Targeted Case Management Assessment within 6 months of employment.
- Access to reliable, personal transportation.
- Maintain required vehicle insurance.
- Meet HCP/CSS waiver policy standards for background checks such as KDHE, DCF Child and Adult Registries for Abuse, Neglect, and Exploitation, KBI criminal history check, and/or Motor Vehicle Report.
TCM will serve as a 1099 Independent Contractor.
