- Schedule: Monday-Friday, no weekends or nights
Job Requirements
- At least 1 year of experience in a Skilled Nursing or Home Care setting preferred
- Ability to travel to local skilled nursing facilities or patients' homes when necessary
- Strong communication and interpersonal skills
Responsibilities
- Develop and implement nutritional care plans for participants
- Collaborate with medical staff to ensure comprehensive care
- Provide nutritional counseling and education to participants and their families
- Monitor and evaluate participants' progress and adjust care plans as needed
