Job Details
- Location: Minneapolis, MN
- Duration: 13 weeks
- Start Date: February 10, 2025
- Shift: 8-hour days, Monday to Friday
- Shifts per Week: 5
- Work Environment: On-Site
- Licensed Master Social Worker (LMSW) certification
- Minnesota license required
- Experience with discharge planning preferred
- Must be vaccinated
- Business casual attire
- Manage roughly 20-25 cases focusing on discharge planning
- Utilize Epic and Telmediq systems for case management
- Provide social work services primarily in the cardiac and renal units
- Float coverage across various units as needed
