Urgent need for a Social Worker -LMSW for one of our client which is a Hospital.
- Job Location: Maple Grove, MN 55369
- Shift: Days(8:00 AM - 4:30 PM)
- Duration of Contract: 13 weeks Extendable
- Weekend Requirements: Every 3rd Weekend
- Guaranteed Hours: 40hrs/week
- Minimum years of experience: 2 years
Pay rate: $53/hr
Job Description:
- Assisting clients in receiving services by telephone and in person.
- Getting clients involved in beneficial activities.
- Assessing clients and gathering relevant information.
- Offering information and supporting clients and their families.
- Contacting and making referrals to other agencies and services.
- Maintaining accurate records and preparing reports.
Position Requirements:
- 2 Years Hospital Social Working Experience - Required
- Acute Care (Hospital) Discharge Experience- Required
- Adult and Geriatric Experience - Required
- BSW, CSW, or MSW - Required
- BLS - Required
- Must have Social Worker License in MN
- Must have MN State License.
